Please use this form to report your Covid test results. If you used a school supplied test kit, ALL RESULTS (positive, negative, invalid) need to be reported.

If your child has been identified as a close contact of a person who has tested positive for COVID-19, you will have been notified by the school. Please complete this form. This is where you will indicate which quarantine option you would like to use.
